We will explain the costs that you can anticipate for your treatment and answer any questions you might have. Our fees are usually 75% lower than traditional surgery.

Fees, Insurance and Payments:

At the time of your consultation, we will explain the costs you can expect for your treatment. Our state-of-the-art Endovenous Radio Frequency Ablation Therapy (RFA) is far less expensive than traditional surgery. For those patients who may require recurring appointments to solve extreme conditions, we also offer discounts when appointments and procedures are packaged together.

Most major insurance companies will cover the costs associated with medically necessary vein removal treatments. Bring your insurance identification card with you so our staff can verify your precise coverage prior to your treatments.

For your convenience, Southern Arizona Laser & Vein Institute accepts most major credit cards to help pay for procedures that are not covered by your insurance provider.

Patient Forms:

To help expedite your check-in process, we offer printable forms for you to complete and bring to your first appointment. Simply click on each applicable form, print out a copy for yourself and bring your completed forms to your evaluation. If you are not able to print these forms in advance, we will give them to you at the office to complete when you arrive.
